IOSH Incident Investigator’s Toolkit - 2 days

IOSH Incident Investigator’s Toolkit is built around an operational scenario to facilitate a variety of inputs and practical exercises, this course cumulatively delivers a highly realistic investigative experience. This course is intended for those members of staff who undertake any aspect of an incident investigation on behalf of their organisation, and for Root Cause Analysis practitioners who are keen to enhance their evidence gathering skills. Participants will become familiar with the use of proven investigation techniques and develop the skills necessary to carry out a professional incident investigation on behalf of the organisation.

Course content:

Significant Investigation Data (SID) Grid – developing a chronological sequence of events designed to help the investigator establish and understand what took place.
Scene management – examination of the incident scene and the gathering, recording and preservation of physical evidence.
Equipment – assessing the relevance of personal protection equipment, tools, machinery, parts and materials to the incident.
Personnel profiles – human factors and the role of individuals relative to the planning, supervision and performance of the activity that led to the incident.
Documents – recovery, review and retention of relevant documents.
Change evaluation – considering ’what changed’ or ‘what was done differently’ that contributed to the incident
Barrier evaluation – understanding the relationship between a hazard and a target & where barriers designed to mitigate the risk either failed or were missing.
Witness interviews – carrying out structured witness interviews and noting statements.

IOSH Investigation Team Leader’s Toolkit - 1 day

Prerequisites: Incident Investigator’s Toolkit IOSH Certificate.

IOSH Investigation Team Leader’s Toolkit builds on the lessons learned during the 2-day Incident Investigator’s Toolkit course and will help develops the skills required to guide a team through the investigation of a workplace incident. Participants gain an understanding of the processes and practices that underpin the successful initiation, set up and management of an incident investigation, and develop the competence to lead an investigation team and deliver successful outcomes for their organisation.

Course content:

Scope – establish the scope and agree the terms of reference.
Logistics – influence investigation logistics-team size and skills profile / location.
Management – set up management processes and supporting systems.
Prioritisation – prioritise lines of enquiry including witness interviews.
Resources – manage the tasking and coordination of resources.
Root cause analysis – drive the root cause analysis and development of corrective actions.
Close out – close down the investigation, compilation of final report and present findings.
